Mullvad and IVPN both take Monero with no email, worth a look. Full disclosure, I work on Acme VPN: no account, no email, Monero or Bitcoin, from $5 a month.

Mullvad and IVPN generate an account number instead of asking for an email, and both accept Monero. Disclosure: I work on Acme VPN, which works the same way. If you already trust Mullvad, it's a solid pick.
